What is Care Coordination?
Care Coordinators help you navigate the health and care system, ensuring you connect with the right teams at the right time. They assist you in managing your healthcare needs by answering questions, making and managing appointments, and providing clear information to help you make informed choices about your care.
If you’re housebound, Care Coordinators can also visit you at home to support your health needs.
How to Contact Your Care Coordinator
Each practice has dedicated Care Coordinators. To reach yours, simply call our reception team and ask to speak with an available Care Coordinator. They will arrange an appointment to discuss how they can best support you.
Who Can Use the Care Coordination Service?
You may be eligible for the Care Coordination service if you:
Frequently attend the hospital (inpatient or Emergency Department)
- Are registered with one of our GP surgeries
2. Are 18 years or older and registered with an Islington GP
3. Frequently attend the hospital (inpatient or Emergency Department)
4. Have two or more of the following concerns:
- Social Issues
- Physical Health Issues
- Mental Health Issues
