Self-Referrals

 

You can now directly refer yourself to some local services without seeing a doctor first.

 

Physio

You can refer yourself to the Physiotherapy services by completing the self-referral form, available HERE.

Once you have completed this form, you need to hand it in to the physiotherapists at the Finsbury Health Centre, or at the Holloway Community Health Centre, 11 Hornsey Street, London, N7 8GG. You can email the service on

musculoskeletal.triage@islingtonpct.nhs.uk 

 

Podiatry

Podiatrists are experts in the care of the foot and ankle, and if you are an Islington resident with foot care problems, you can now refer yourself to the podiatry service without waiting to see a doctor by completing the information booklet and referral form, available HERE.

 

Sicknotes

For illnesses lasting 1-3 days your employer should not require a sick note. For illnesses lasting between 3 and 7 days, your employer should accept a self-certification form, available to download HERE.

 

Antenatal (Pregnancy Care)

If you have recently found out that you are pregnant, you can refer yourself to the antenatal clinics at UCLH and the Royal Free Hospital.

Health Trainers

The Health Trainers are a service which can provide help, motivation, and support to Islington residents who wish to lose wight, give up smoking, exercise more, or make other lifestyle changes. For more information, and to refer yourself, please downoad the health trainers' flyer, available HERE.

 

Expert Patients Programme

The Expert Patients Programme is a free 6 week course for any adult living with one or more long-term health condition.

(e.g. asthma, arthritis, MS, depression, COPD, heart disease, HIV, chronic pain etc.)

The course aims to help you take more control of your health by learning new skills to cope with and manage your condition. Courses are run in English and Turkish.

REACH Intermediate Care Team

The REACH team works in collaboration with older people and their relatives/carers, to promote health and maximise independence in mobility, communication, work, leisure and everyday activities. This work is carried out in their own homes, day centres, residential and nursing homes, or any other appropriate environment.

More information is available HERE, and the team are always happy to discuss referrals/requests for advice with clients themselves, their relatives and their carers.

MEND - Exercise for Children

The MEND campaign for London is a free 10 week community-based programme which aims to help children become fitter, healthier and happier.

Smoking Cessation

The NHS stop-smoking services are directly open to patients who want to quit smoking. The first step is to phone free on 0800 093 9030 or email smokefree@islingtonpct.nhs.uk

Alternatively, patients can self-refer to our in-house smoking cessation service. Please phone up to book an appointment with our smoking cessation advisor Ahmet Caglar. Please mention that it will be your first appointment.

 

Bladder & Bowel Service

Patients above the age of 3 years can now self-refer to the specialist Bladder & Bowel service.

This service sees both adults and children who are experiencing any kind of bladder or bowel problems.

To self-refer, simply call the central booking office on 020 7527 1158
